Community Access Programs

Receive the supports you need to re-engage with your local community. Join our Community Access Group and you will be able to meet with like minded peers, attend local events and participate in local activities. Form friendships in a social and casual environment, experience new activities and places led by group members and increase your independence all while having fun.

Overnight Respite

Our In-Home Respite Service is very beneficial for the aged and people living with dementia as it maintains a familiar environment in which they are comfortable with.

Together, we can develop a flexible, tailored care plan so that you can rest with ease knowing that everything will be taken care of.

Living With Dementia Program

If you are living with early stage dementia or are a family carer for someone with dementia, you are welcome to join our early intervention program.

Description:

If you are living with early stage dementia or are a family carer for someone with dementia, you are welcome to join our early intervention program.

The purpose of this program is to reduce the sense of isolation, distress and depression and to increase the self esteem of people with, or caring for someone with dementia.  The program also aims to increase participants knowledge of local services and gain positive perceptions of dementia.

The program is delivered over a seven week period, two hours a day on a weekly basis.

During the program, participants have an opportunity to:

  • Tell their story of dementia
  • Reflect on the impact of diagnosis
  • Develop an understanding of the importance of self-care.
  • Enhance skills for communicating with others.
  • Learn techniques for problem solving.
  • Extend their knowledge about legal and financial issues.

Participants must have a diagnosis of dementia or be accompanying a person with a diagnosis of dementia.

When: Wednesdays – April 3rd – 15th May 2019
Time:  10am to 12pm (light refreshments included)
Venue:  Seventh Day Adventist Church, 83-93 Phillip Street, Chinderah
Fee:  No charge

Wellness emphasises a person's needs, aspirations and goals. It also acknowledges and builds on a person's strengths. Lifebridge offers individual support, with the focus on greater independence and quality of life.  We will support you to reach your personal goals and aspirations.

You have the choice of after-hours services, weekend support or short term respite for carers needing a break.

Reablement Support

Reablement is about assisting people to regain functionality capacity and improve independence.  Similar to rehabilitation, it is goal-orientated and aims at full recovery where possible.

Take A Break

Take a Break is social group for people who have a cognitive impairment diagnosis. Group members determine the locations they want to visit and the activities they want to do.

Description:

Take a Break is social group. Group members determine the locations they want to visit and the activities they want to do.

In Take a Break, you will join a friendly and welcoming group and will develop friendships with other members. Take a Break is also a great opportunity to stay connected with the local community, events and activities.

Day: Wednesday / fortnightly
Time: TBC
Location: Chosen by group members

Everyone is encouraged to express their ideas and suggestions for upcoming outings in the local community.

We have men’s groups, ladies groups and mixed groups.

Day: Monday – Friday
Time: 9am – 4pm
Venue: Locations determined by group participants
