COMMONWEALTH HOME SUPPORT PROGRAM

The Commonwealth Home Support Programme (CHSP) is a government-funded initiative that provides entry-level support to help older Australians live safely and independently at home while staying connected to their communities. Designed for the healthy and active aged, as well as those who need just a little extra assistance, the CHSP helps you continue to live life to the fullest. Whether you need a small amount of assistance or are simply looking for ways to stay socially connected and engaged, this program can provide the right support to suit your needs.

Often, just a little bit of help in the right area—such as maintaining your home, accessing social opportunities, or participating in healthy living activities—can make daily life easier, enhance your independence, and support a more fulfilling lifestyle. The CHSP is not intended for those with complex or intensive care needs, who may benefit from other aged care programs like Home Care Packages (HCP) or Residential Aged Care.

Designed to promote independence and social connectedness, Lifebridge can work with you to provide support that is tailored to your individual goals and preferences through the CHSP.

As a government subsidised entry level program, you may be eligible to receive these services and only need to make a small financial contribution based on the services you receive.

Eligibility for CHSP Services:

You may be eligible to receive CHSP services if you:

  • Are aged 65 years or over, or aged 50 years or over and identify as Aboriginal or Torres Strait Islander
  • Require assistance with everyday activities to support your independence and social connections

CHSP SERVICES

Lifebridge can work with you to tailor a range of services to support you to live independently. Services available through Lifebridge inc

Domestic Assistance:

  • General House Cleaning
  • Linen services
  • Unaccompanied Shopping (delivered to home)

Social Support Individual:

Assistance provided by a support worker either in the community or in your home to support social contact and/or company to participate in community life. Activities may include:

  • Assistance with shopping and running errands
  • Transport and support to attend appointments

Social Support Groups:

Social Support Groups provide you with opportunities to engage in social interactions through structured activities in the community or at our day centre, The Cottage. These group-based activities are designed to foster social connection, promote independence, and enhance well-being through physical, cognitive, and emotional stimulation.

Respite Care:

To support and maintain relationships between you and your carer. Lifebridge can provide quality respite care in your home so that your carer may take a break.

All CHSP services can be accessed on a short-term, occasional, or ongoing basis, depending on your individual needs. The program is designed to enhance the quality of life for older people by helping them remain in their homes longer, participate more actively in their communities, and maintain or improve their physical, emotional, and mental well-being. By promoting independence, the CHSP supports individuals in enjoying a fulfilling life while delaying or avoiding the need for residential care.
