IN HOME SUPPORT TO FOSTER INDEPENDENCE

Our in-home care services are designed to meet your unique needs, enabling you to live comfortably and confidently in your own home. From daily living assistance to personalized health and wellbeing programs, our skilled team works with you to align your care with the goals outlined in your NDIS plan.

SHORT TERM ACCOMODATION (STA)

Short Term Accommodation (STA) also known as respite provides people with a disability the opportunity to have some time away from home and for their families and carers to have a break.  Our Pottsville Short Term Accommodation House is open 7 days a week, 24 hours a day and is beautiful blend of residential and coastal living. Compliant with Disability Service Standards, it is suitable for people of all support needs.

Engaging Group Activities to Achieve Your Goals

Lifebridge offers dynamic group activities aimed at building skills, fostering social connections, and enhancing quality of life. Whether you’re looking to stay active, learn something new, or connect with your community, our programs are here to support your journey.

Cooking group

Health and Wellbeing

Start the week with a healthy meal.

This group will help to bring out your inner chef as you learn how to create a delicious and healthy meal to share.

Community art program

Creative Relationships

Get in touch with your creative side.  Start the day with an activity in the community, followed by morning tea and creative art and craft in the afternoon.

Sailing and fishing group

Sailing and Fishing – is a program that offers an additional level of support to assist people of all abilities to get into sailing.  From the many rivers, estuaries, and secret fishing spots of the Tweed Coast, Lifebridge will take you to all the fishing hot spots.

Music group

Music and Culture – Let your multi-talented and multi-instrumentalist tutor guide you through a new and eclectic program of dance, singing and rhythm and movement.

Immerse yourself in local culture as you explore some of the regions festivals, tourist attractions, art galleries, cafes and restaurants, museums and historical villages.

Active and Sports Club – Let your personal trainers take you through a 12-week workout of sporting activities.

This program has been designed with a diverse range of sporting pursuits including golf, lawn bowls, horse riding experience, table tennis, basketball and the Rock Hoppers Ramble Walk.

Mystery Bus Tour – Why not sign up for our Saturday Mystery Bus Tour (once a month) with a new theme each month exploring our local community.  Have fun along the way with bus Bingo and other fun activities.

REPORTING ON YOUR GOALS

To assist you in your NDIS Plan reviews, we will also provide you with progress updates.  These updates will allow you to demonstrate to the NDIS how you are progressing with your goals.  We will include a summary of supports provided to you; how the support has helped you to work towards your goals, any barriers that you may encountered during the plan period and adjustments made; risk identification and recommendation of additional supports, where required.
