The types of roles we have at Lifebridge are:
Aged and NDIS Services
Provision of professional, high quality and safe supports to Lifebridge customers at home, in accommodation, in groups, in the community and one on one services.
Provision of support to Lifebridge customers in accordance with their care plans and goals.
Home Support Worker
Aged Care Services
Provision of personal home care support enabling our customers to maintain independence and remain in their home.
Provision of social support and community access which may include respite, transport to appointments, shopping, social activities.
Provision of housekeeping assistance including: dusting, vacuuming, sweeping, mopping, meal preparation, making beds, laundry, ironing, and cleaning bathrooms.
Customer Care Manager
Aged Care Services HCP and CHSP
Responsible for assessing, managing and coordinating services and supports for a case load of Home Care Package customers. You will have the opportunity to work flexibly from home, our Kingscliff office and on-site with customers.
Develop person centred care plans to support our customers needs and goals, indentify, coordinate, implement and monitor supports and services for customers in line with care plans and budgetary constraints. Develop and manage relationships with customers, carers, families and community service providers.
We have a range of administration functions within our Aged Care, Community Services, Business Services, Finance and Administration and Executive Teams including:
